Are you a new patient to Delicate Periodontics & Implant Dentistry? We are glad you found us, and we want to provide you with a smooth experience from beginning to end. To help us accomplish this, we’ve supplied helpful information below. Please take a few minutes to review it and and use it to help you adequately prepare for your visits with us. If you have questions about your appointment, we invite you to reach out to our team, and we’ll gladly help however we can.
Your First Visit
When you arrive for your first appointment, you will be warmly greeted by our front office team members, who can give you a short tour of our periodontal office. Then, you will meet with Dr. Delica for a one-hour consultation. A week following this initial visit, you’ll return for a case discussion, at which time, we will have all the information we need to present a detailed treatment plan with options, costs, timelines, and more. After this point, we can schedule treatment and move forward.
New Patient Forms
When you make your first appointment with us, we will get certain information from you, including your email address. Then, we will email some forms to you and ask you to complete them before you arrive. Please take a little bit of time to fill in this paperwork. That way, you will spend less time in our waiting room, and we will be able to do everything on our end to ensure an efficient visit for you.
